Comments:

An unannounced monitoring inspection was conducted today from 4:00pm-6:10pm. There were 40 children directly supervised by 5 staff at the beginning of the inspection. The physical plant, programming, menus, 5 staff records, 6 children?s records, 12 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed reading books, doing homework, washing hands, eating snack which consisted of milk and a fresh apple, and engaging in gross motor exercise playing in the gym. Positive interactions between children and staff were observed during this inspection. There was an adequate supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #:	22VAC40-185-130-A
Description:	Based on record review, 1 of 6 children's records reviewed did not contain documentation, signed by a physician, physician's designee or health department official, that the child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: Child #6's record did not contain the immunization record signed by a physician, physician's designee, or health department official.
Plan of Correction:	I will request the immunization record from the parents.
Standard #:	22VAC40-185-140-A
Description:	Based on record review, 1 of 6 children's records reviewed did not contain a physical examination obtained either before the child's first day of attendance or within one month after attendance. Evidence: Child #6 (Enrollment Date: 4/24/19) did not have a physical examination on file.
Plan of Correction:	I will request the physical from the parents.
Standard #:	22VAC40-185-510-A
Description:	Based on review of medications, 3 of 12 medications were on site without a parental medication authorization form. Evidence: Child #7, Child #8, and Child #9 had emergency medications on site and did not have a medication authorization form signed by each child's parent.
Plan of Correction:	I will have parents sign the medication authorizations. I will also print more copies to have on hand when needed.
Standard #:	22VAC40-185-510-E
Description:	Based on review of medications, 5 of 12 medications reviewed were not labeled with the child's name, the dosage amount, and the times to be given. Evidence: 5 nonprescription medications were observed in the medication box and were not labeled with the child's name, dosage amount, and times to be given.
Plan of Correction:	I will label prescriptions with all required information.
