Comments:
An unannounced mandated renewal inspection was conducted 2/13/2020 There were 75 children present with 27 staff. The inspector arrived at 8:30 A.M. and departed the program at 12 P.M. The inspector reviewed nine children's records and staff's records were reviewed at a earlier inspection and ten medications.

Observed the children during arrival, free play, breakfast, departure for a field trip, circle time, story time and outdoor time.

Standard #: 22VAC40-185-130-B
Description: Based on review of nine children's records, the center failed to ensure that all immunizations were updated every six months for children under the age of two years.

Evidence:
1. Child # 1 most recent immunization was completed on 6/18/2019 and the child is under the age of two years. The immunizations were required to be updated by December 2019.

Plan of Correction: The FSW will contact the parent to obtain an updated immunization record. All immunizations will be monitored for updates.

Standard #: 22VAC40-185-270-A
Description: Based on observation of the classrooms, the center failed to ensure that the areas and equipment was maintained in a clean, safe and operable condition.

Evidence:
1. In classroom A the strip of the blind that was locked in the kitchen center was cracked and posed a hazard to the children in care.

Plan of Correction: The site coordinator will remove the cracked strip today and will have staff to monitor all classroom areas.

Standard #: 22VAC40-185-510-J
Description: Based on observation and review of medications, the center failed to ensure that all medications were locked.

Evidence:

1. In classroom H there was a prescription medication that was not stored in a locking safety sack. The medications was in a ziplock bag in the first aid backpack.

Plan of Correction: During the inspection the medication was placed in a locking safety sack. All medications will be locked.
