Comments:
An unannounced monitoring inspection was conducted today. There were 76 children in care, with a total of 13 staff members assigned to their direct supervision. Children's records, administrative records, emergency preparedness items, staff record, staff training, background checks, the indoor/outdoor physical plant, classroom programming, and classroom interactions were inspected today.

All classes were actively engaged in a large variety of creative activities, including: creating theme based art projects, "flying" airplanes and other child created transportation activities, making a "campsite", listening to and discussing stories, playing outside, and eating snack. All classrooms and the hallway are decorated with a variety of the children's art projects.
Dismissal from the program was also observed today.

Violations:
Standard #: 22VAC40-185-80-A
Description: Based on a review of administrative records, it was determined that one class did not keep an accurate written record of children in attendance today.
Evidence:
In the Pandas classroom, there were 13 children present and in care. However, the record of attendance indicated a total of only 12 children present and in care.

Plan of Correction: One child is not usually here on Thursdays. Her name will be added to the attendance.

Standard #: 22VAC40-185-270-A
Description: Based on observations, it was determined that one piece of equipment has not been maintained in a safe and operable condition.
Evidence:
On the playground, the Inspector observed that on the adult picnic table, two corners of the top of this plastic table have separated from the foundation and are sticking up. This separation poses a pinching hazard for the children's fingers.

Plan of Correction: I called maintenance immediately during the inspection to request the repair.

Standard #: 22VAC40-185-280-B
Description: Based on observations, it was determined that two potentially hazardous substances were not stored in a locked place.
Evidence:
A container of table cleaning wipes was stored in an upper level unlocked cabinet inside the Fish classroom and inside the Pandas classroom.

Plan of Correction: Both items were immediately moved and locked during the inspection.

Standard #: 22VAC40-185-330-B
Description: Based on observations, it was determined that one fall zone area on the playground does not have an adequate level of resilient surfacing.
Evidence:
At the bottom of the slide, the mulch has worn down to a level of dirt.

Plan of Correction: The children raked the mulch from other areas of the playground into the space at the bottom of the slide. We will continue to rake it when needed, and re-evaluate in the spring to determine if more mulch is needed.
