Comments:
Today an unannounced Monitoring Inspection took place between the hours of 11:45am and 2:05pm. There were 4 classrooms observed with a total of 27 children with 6 staff within the supervision guidelines. The children were observed eating lunch (chicken fajitas with whole wheat wraps, green beans, applesauce and milk) washing hands and napping. Diaper changing procedures were observed. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. There was 1 medication and authorization reviewed today. Positive interactions between the children and staff were observed. Violations:
Standard #: 22VAC40-185-500-A
Description: Based on observation, staff did not wash their hands before and after diapering children.
Evidence: Staff in the toddler classroom were observed changing two children's diapers. The staff completed the first child's diaper change, handed the child off to another staff person, documented the diaper change on a clipboard and proceeded to change the second child's diaper.

Plan of Correction: I'll talk to the staff today and retrain them on hand washing.

Standard #: 22VAC40-185-500-B
Description: Based on observation, diapering surface were not cleaned with soap and sanitized after each use.
Evidence: Two children in the toddler classroom were observed having their diapers changed. The diaper changing surface was not washed and sanitized after each use.

Plan of Correction: I'll talk the staff about sanitizing today.

Standard #: 22VAC40-185-520-A
Description: Based on observation, over-the-counter skin products were kept beyond the expiration date.
Evidence: Child #6 had two over-the-counter diaper cream observed at the infant changing station. Cream #1 expired 1/2008 and cream #2 expired 7/2019.

Plan of Correction: We'll talk to mom today to see if she wants to provide new diaper creams if the child needs them.

Standard #: 22VAC40-185-520-C
Description: Based on observation, diaper ointment was not labeled with a child's name.
Evidence: Child #6 has diaper ointment observed in the toddler classroom. It was not labeled with the child's name.

Plan of Correction: We'll label it today with some tape. I'll remind the staff to double check labeling.
