Comments:
An unannounced Monitoring Inspection was conducted today from 3:40pm to 4:35pm. There were 33 school-age children directly supervised by 3 to 4 staff. The physical plant, 2 staff records, 5 children?s records, 1 child's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed arriving to care and going outside to play. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Standard #: 22VAC40-185-130-A
Description: Based on review, the facility did not ensure that they obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence: Reviewed 5 children's records and found that Child 5 and 6 did not have documentation of immunization records.

Plan of Correction: Will obtain from parents.

Standard #: 22VAC40-185-140-A
Description: Based on review, the facility did not ensure that each child has documentation of a physical examination by or under the direction of a physician.
Evidence: Reviewed 5 children's records and found that 2, Child 5 and 6, did not have documentation of a physical examination.

Plan of Correction: Will obtain from parents.

Standard #: 22VAC40-185-60-A
Description: Based on review, the facility did not ensure that children's records were maintained with the required information.
Evidence: Reviewed 5 children's records and found that Child 1 and 2 did not have documentation of parent 2, Child 3 did not have documentation of parent 2's work information, and Child 5 and 6 did not have work numbers for both parents. Previously cited on 9/12/19.

Plan of Correction: Will obtain information from parents.

Standard #: 22VAC40-185-540-E
Description: Based on observation, the facility did not have a working battery operated radio on site for emergencies.
Evidence: Observed and identified from staff interviews that they had a battery operated radio, but no batteries. Previously cited on 9/12/19.

Plan of Correction: They are on their way.

Standard #: 22VAC40-185-560-F
Description: Based on observation, the facility did not ensure that when food is provided the menu for the current one-week period shall be dated, posted and substituted food must be indicated.
Evidence: Observed that the facility only had the school menu posted and not the AlphaBEST snack menu.
