Comments:
An unannounced monitoring inspection was conducted on 02/20/2020. There were 13 children, ages 3 years-5 years, and 3 staff members present during the inspection. The inspector reviewed 5 children's records, the on-site staff record requirement for 3 staff members and 2 medications during the inspection. The children were observed in the following activities: arriving to the program, having breakfast, during circle time, listening to a special guest speaker, and participating in an art project. The inspector reviewed the emergency supplies during the inspection. The inspector discussed areas and equipment with the staff. Standard #: 22VAC40-185-290-3
Description: Based on observations and discussion with the staff, the facility failed to ensure that in areas used by children of preschool age or younger electrical outlets shall be protective covers.

Evidence: The electrical outlet strip behind the teachers desk had 3 outlets that were not covered. The children in the room were of preschool age.

Plan of Correction: The outlets will be covered today.

Standard #: 22VAC40-185-510-C
Description: Based on medication review and discussion with the staff, the facility failed to ensure that the parent's authorization to administer medication included a duration of the parent's authorization.

Evidence:
1. The medication authorization form for Medication A did not include a duration of the parent's authorization.
2. The medication authorization form for Medication B did not include a duration of the parent's authorization.

Plan of Correction: The parent's will be asked to provide a duration today.
