Technical Assistance:
Discussion was held regarding medication and playground wood fences and areas.

Comments:
An unannounced renewal inspection was conducted from approximately 9:00 am through12:30 pm. During the inspection, 57 children were in direct care of 7 staff members. Ratios were in compliance. Children were observed during snack time, engaged in group activities, circle time, exercising during outdoor time, manipulatives and engaged with staff. The center was clean, organized and contained a varied supply of developmentally appropriate materials. A selection of 4 staff and 4 children records were reviewed. The physical space, evacuation drills, attendance records, injury reports, a sampling of medications, required posted documents and emergency supplies were also reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review, not every staff member submit documentation of a negative tuberculosis (Tb) screening that was completed within 12 months prior to employment.
Evidence:
1) The record for Staff 4 (doh 09/23/2019) did not contain a current Tb screening completed within 12 months of employment. The most recent Tb screening was dated (07/23/2018).

Plan of Correction: We will ask the Staff to go complete a Tb screening.

Standard #: 22VAC40-185-240-A
Description: Based on record review, staff did not receive mandatory training by the end of their first day of assuming job responsibilities.
Evidence:
1) The record for Staff 1 did not contain documentation of having completed mandatory orientation training and training on recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse as required.
2) The record for Staff 3 did not contain documentation of having completed training on recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse as required.

Plan of Correction: We will complete the required training by tomorrow.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on record review and interview, the center failed to ensure all staff completed fingerprint background checks.
Evidence:
1) The record for Staff 1 did not contain results of a current Fingerprint check.
2) Staff 1 stated that she could not find the fingerprint results in her file.

Plan of Correction: I will make the appointment today to complete the fingerprint check.
