Comments:
An unannounced monitoring inspection was conducted on Tuesday, November 12, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 10:00 AM and concluded the inspection at 3:45 PM. The census for today's inspection consisted of 105 children. During the inspection, children were playing with push and pop up toys, playing on the playground, participating in dance classes, working with letters of the alphabet, washing hands, preparing for lunch, eating lunch, listening to stories, and napping. The classrooms are decorated with children?s art work and equipped with age and stage appropriate materials. Children's equipment, learning materials, indoor and outdoor space, restrooms, first aid kit and the nonmedical emergency supplies were reviewed. Medication is administered and medication, authorization forms and allergy plans were reviewed. The last 10 injury reports were reviewed as well as the center?s injury prevention plan which was updated July 2019. Ten children's records and ten staff records, annual fire and health inspections, fire drill and shelter in place drill logs, daily attendance, and all required postings were reviewed.

Two members of administration were available for the inspection and were present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22VAC40-185-160-A
Description: Based on a review of staff records, the center did not ensure that each staff member shall submit
documentation of a negative tuberculosis screening (TB) no later than 21 days after employment or volunteering.

Evidence:
1. Staff #2 (date of hire 10/07/2019) did not have documentation of a TB screening.
2. The director confirmed that the screening was not in the record for staff #2.
