Technical Assistance:
Discussed the following with management; the Adam Walsh link for all out of state staff in the past 5 years.
Encouraged using just the state model form for the child record information.
Discussed methods to track immunization records in the future, and recording injuries. The state model form for injuries was provided.
Discussed the requirements for subsidy, the use of the allergy action form.

Comments:
An unannounced monitoring inspection was conducted from 8:50 am-12:30. There were a total of 11 children with 4 staff members present. Children were observed engaged in group activities, diapering and hand washing. A selection of staff and children records, and other documents were reviewed. There was no medications this date. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records, the center has not maintained a completed record for staff hired after March 1, 1996, documenting that two or more references as to character and reputation as well as competency were checked before employment.
Evidence: The following staff only have one reference on file; staff #1, date of hire 12/23/2019 and staff #2, date of hire 1/17/2020.

Plan of Correction: We will ask for new references from the 2 staff and in the future references will be obtained prior to start date.

Standard #: 22VAC40-185-550-M
Description: Based on a review of injuries, the center is not maintaining a written record of children's serious and minor injuries with all the required information.
Evidence: The center is completing a 'boo boo report ' with the name, date, what happened and staff and parent signature. The following required information is missing; time of injury, staff present and treatment, date and time parent notified, any future action to prevent recurrence, and documentation of how parent was notified.

Plan of Correction: We will use the state model form to record injuries.
