Comments:
An unannounced renewal inspection was conducted on 2/19/20 from 9:50 am until 2:15 pm to review licensing standards. At the time of the inspection 117 children and 28 staff were present. The sample size consisted of ten children's records, ten staff's records, two children's medications and supporting documents, and four diaper ointments. Children and staff were observed during free indoor and outdoor play, educational and artistic activities, story time, cleaning, singing, lunch, nap, transitions and behavioral management. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on observation and interview, the center failed to ensure that the center's activities and services were maintained in compliance with the center's own policies and procedures that are required by the standards.

Evidence:

1. For prescription medication the center's policies state the following: "Prescription medications require a note signed by the family and a written order from the child's physician. A specific form is required and can be located in the center office."
2. The medication form for child m2 was not signed by the child's parent.
3. The director verified they did not have a signed parent form for child m2.

Plan of Correction: The parents of child m2 were given the medication form to sign. In the future when the medication forms are turned in the parents will be asked to stay until the documents are reviewed for any errors.
