Comments:
An unannounced monitoring inspection was conducted today from 9:50am to 1:20pm with the Director. There were 51 children in care, ranging from four months to five years of age, supervised by nine staff. The children were observed having structured free play in centers which consisted of dramatic play, coloring, water play, table top manipulatives, and painting. Six children?s records and five staff records were reviewed and were in compliance. All of the center's staff have current certification in CPR and First Aid as well as Daily Health Observation Training. Eleven medications and authorization forms were reviewed and the center has five staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed and were in compliance. The most recent evacuation drill was documented 1/31/20, and the most recent shelter-in-place drill was documented 9/17/19. The most recent Fire Inspection on file was dated 10/15/19 and the most recent Health Inspection on file was dated 10/3/19. Lunch today was chicken nuggets, green beans, applesauce, and milk.

Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 2/24/20. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 22VAC40-185-510-C
Description: Based on review of 11 medications and documentation and interview with the Director, the center did not follow medication policies in that nonprescription medication was not consistent with the manufacturer's instructions for dosage. Evidence: The parent's authorization for Child A's Equate brand pain and fever medication dosage stated to give 3 tabs, however, the manufacturer's instructions for the child's age stated 2.5 tabs.

Plan of Correction: Medication was at the center as a precautionary measure, but was never administered, and therefore the incorrect dosage was never given.

Standard #: 22VAC40-185-510-N
Description: Based on review of 11 medications and documentation and interview with the Director, a medication was not returned to the parent, or authorization from the parent renewed, within 14 days after the authorization expired. Evidence: The Licensing Inspector observed an Equate brand children's pain and fever medication on site for Child A with parent authorization that expired 2/3/20.

Plan of Correction: Parent was called to pick up the medication since the authorization form was out of date. All medication labels and documents will be thoroughly reviewed by the manager on a regular basis.
