Discussed with the Director using the new Sworn Statement forms.

Comments:
An unannounced monitoring inspection was conducted today between 10:00 am and 2:30 pm. There were 87 children present with 16 staff members. The physical plants, children's records, staff records, attendance records, evacuation logs, medication records, emergency plans, menu's and emergency supplies were inspected. The children were observed singing and reading stories. They were also observed hand-washing and eating lunch. Lunch consisted of French Toast Bites, Turkey Sausage Bites, Home Fried Potatoes, Apples and Milk. Areas of non compliance were cited on the Violation Notice. Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records, the center failed to ensure that each child enrolled will have documentation of a physical examination in their records by the 30th day of attendance.

Evidence:
Child #2 and #4's records did not have documentation of physical examinations by the 30th day of attendance.

Plan of Correction: I will contact the parents regarding documentation.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
In classroom #6 there was a soap labeled keep out of reach of children in an unlocked area accessible to children.

Plan of Correction: The item was removed during the inspection.

Standard #: 22VAC40-185-280-H
Description: Based on observation of the physical plant, a purse was found on the floor, accessible to children.

Evidence:
In classroom #2, a purse was found on the floor accessible to children.

Plan of Correction: The Director removed the purse during the inspection.

Standard #: 22VAC40-185-510-C
Description: Based on a review of medication, the center did not ensure that procedures for administering medication shall include methods to prevent the use of outdated medication.

Evidence:
1.There was and Epinephrine Pen with an expiration date of August 2019 and an EpiPen, Jr. with an expiration date of December 2019. 2. The Center Director confirmed that the two medication were expired and had not been returned to the parents.

Plan of Correction: The medications will be returned by 02/25/2020/

Standard #: 22VAC40-185-560-G
Description: Based on observation, food brought from home was not properly labeled.

Evidence:
A plastic container of toast on the children's cubbie was not labeled with the child's name or the date.

Plan of Correction: Teacher was aware of who the toast belonged to and placed the container in the child's backpack to be returned home.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a review of staff records,, the center failed to obtain one out of state background check.

Evidence:
Staff #2 resided out of state within the last five years, the out of state background check for one of the states was not completed.

Plan of Correction: I will send out request as by 2/28/2020.
