Technical Assistance:
It is highly recommended that the school use the Division's model forms to record emergency drills.

Comments:
An unannounced monitoring inspection was conducted today. There were 60 children in care, with a total of 12 staff members assigned to their direct supervision. Children's records, staff training, background checks, staff records, the physical plant, emergency preparedness items, administrative records, children's medications, classroom programming, and staff:child interactions were inspected today.

The children were observed playing soccer in PE class, engaging in active creative play, painting pictures, singing songs, practicing counting skills and letter/word recognition skills, and enjoying classroom dress up activities that coordinate with the "Our Senses" theme of the week.

Violations:
Standard #: 22VAC40-185-150-A
Description: Based on a review of children's health records, it was determined that one record does not include a physical exam report documented on the current VA Department of Health form.
Evidence:
Child record E does not include a physical exam recorded on the VA Department of Health form. This child's physical was documented on a health form from another state and does not include all information that is required on the VA Department of Health form.

Plan of Correction: I will ask the parent to have the VA form completed.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that two records do not contain all required information.
Evidence:
1 - Child record B does not include: the current information for two emergency contact people; current "authorized pick ups" information.
2 - Child record E does not include the parent's work phone number.

Plan of Correction: Each parent will be asked for the information.

Standard #: 22VAC40-185-240-A
Description: Based on a review of staff training documents, it was determined that there is no documentation to show that two staff members completed all required components of orientation training by the end of their first day of assuming job responsibilities.
Evidence:
There is no documentation to show that staff B and staff C have each completed "Recognizing Child Abuse and Neglect" training.

Plan of Correction: Both staff did the training many years ago when they first started. I will make sure both staff do the training again and place their certificates in the file. All staff will be required to do the training again and place their certificates in each file.
