To be sanitized, means the sanitizing solution is sprayed on the surface and allowed to air dry for at least a minute before being wiped dry with paper towels.
Documenting training to qualify staff for lead teacher positions in addition to documenting programmatic experience.

Comments:
An unannounced monitoring inspection was conducted from 10:00 am - 12:30 pm. There were 27 children, ranging from one year to six years of age, supervised by seven staff. Children were observed eating snack, and completing Montessori work. Snack served today was goldfish, apples and water. Five children's records and four staff records were reviewed and in compliance. The first aid kit was observed and in compliance. Five medications and authorization forms for three children were reviewed. The last documented emergency escape drill was conducted: 1/17/20 and the last documented shelter-in-place drill was conducted: 11/27/19. Violations:
Standard #: 22VAC40-185-510-C
Description: Based on review of five medications and authorization forms, it was determined that not all procedures for administering medication were followed. Evidence: Child #1 had a long term medication authorization form that was signed by a physician but did not have the parent's signature, and a long term medication form for a second medication that was signed by the parent but did not have the physician;s signature.

Plan of Correction: Will obtain new parent and physician signatures for all long term medication.

Standard #: 22VAC40-185-510-E
Description: Based on review of five medications, it was determined that not all medications were labeled with the child's name, the dosage amount, and the time or times to be given. Evidence: Two emergency medications for child #1 and child #3 were not labeled with the child's name, the dosage amount and the time or times to be given. One over-the-counter medication for child #1 was not labeled with the child's name.

Plan of Correction: Although medication was in individually, clearly marked bags per child, we labeled all individual medications and we will ask the parents to provide prescription labels for the emergency medications.
