Comments:
An unannounced renewal inspection was completed on 02/12/2020 from 3:20P.M.-5:30 P.M. At the time of the inspection 17 children were in care with two staff, one site manager, and one administrator. Five children records, four staff records, two agents background checks, emergency evacuations, first aid kits, emergency supplies, four injury reports, parent handbook, injury prevention plan, playground safety plan, fire inspection, health inspection, and posted required information were reviewed. Children were observed during arrival, free choice activities that included games, books, art activities, handwashing, snack time, behavioral guidance, gross motor time in the gym, and dismissal. Exceptions to compliance are noted on the violation notice.

Violations:
Standard #: 22VAC40-185-340-F
Description: Based on observation and interview, the center failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.

Evidence:
1. On 02/12/2020 the licensing inspector observed that six children, between the ages of 5-10, were out of the sight and supervision of staff for approximately two to three minutes.There were three children observed at the boys restroom standing inside the doorway, and three girls in the girls restroom washing their hands and using the restroom. There was not a staff member present.
2. Staff #4 exited a classroom and saw the children in the hallway. Staff #4 verified that she did not know the children were sent down from the cafeteria to wash their hands. Staff #4 stated she had one child with her and they were retrieving items from a storage space for snack.
3. Staff #2 stated she sent the children to wash their hands for snack. Staff #2 thought that Staff #4 was in the hallway supervising handwashing.

Plan of Correction: The site manager will speak with each staff member to discuss the issue so that each is aware of the concern and the importance of letting each other know where they are when they leave the room. Also, each staff will be told that they must make face-to-face contact with another staff person when sending children to another location to ensure proper supervision at all times.

Description: Based on review of injury reports, the center failed to ensure that all injury reports contained all required information.

Evidence:
1. In injury report dated 02/07/2020 did not contain documentation of how the parent was notified, and the time the parent was notified.
2. An injury report dated 01/14/2020 was missing the time the parent was notified.
3. Staff verified that the injury reports were missing documentation of how the parent was notified, and the time the parents was notified.

Plan of Correction: The site manager will review will all staff how to complete an injury report and review all components.
