Technical Assistance:
Clear site and sound supervision was discussed as it related to classroom arrangement for the 2 and 3 year old room.

Comments:
An unannounced Monitoring Inspection was conducted on February 26, 2020 from approximately 11:00 am. until 2:25 pm. Upon entrance there were 40 children and 8 staff present with children. The sample size of records reviewed consisted of 6 children's records and 3 staff records. The center is not administering medications. The playground was not inspected due to inclement weather. Children were observed during their morning routine to include singing, dancing and freely playing.
The results of the inspection were reviewed and verified with the Center Director during the exit interview. Areas of noncompliance are noted on the violation notice. If you have any questions, contact your licensing inspector at (757) 404-0568.

Violations:
Standard #: 22VAC40-185-40-J
Description: Based on documentation review and staff interviews, the licnesee did not ensure the injury prevention procedures shall be updated at least annually based on documentation of injuries and a review of the activities and services.

Evidence: The Center Director confirmed there was not updated injury prevention plan available for review.

Plan of Correction: The Center Director will review all injuries and will adjust the plan annually according to the injuries.

Standard #: 22VAC40-185-500-B
Description: Based on observation and staff interviews, the licensee did not ensure that all requirements for diapering were followed.

Evidence: The following was reviewed during the inspection and determined inadequate.
1)The diapering area requires a nonabsorbent surface for diapering or changing. For children younger than three years, this surface shall be a changing table or countertop designated for changing.
a) In the 2 1/2 to 3 year old room there are two children (child #2 and child #3) who are two years old and the center staff confirmed they are changed while standing up as there is not a diaper changing table or surface in the classroom.
b) In the Pre-Toddler room the diaper pad has two rips on the top of the diaper pad and therefore it is no longer nonabsorbent.
2) There is not a leakproof covered receptacle for soiled linens in the Pre-Toddler room, 2 1/2 to 3 year old room and the infant room. Center Director confirmed theses items were not in the classrooms.

Plan of Correction: The center director move children who are not completely potty trained to another classroom and replace the diaper changing pad and purchase bins for soiled linens. Children younger than 3 will be changed on diaper table until classroom assignment has been done effective immediately.

Standard #: 22VAC40-185-540-A
Description: Based on observation and staff interviews, the licensee did not ensure that on each floor of each building used by children there was a complete first aid kit.

Evidence: The first aid kit on the second floor did not contain triangular bandages.

Plan of Correction: The Center Director will purchase additional triangular bandages today.

Standard #: 22VAC40-185-550-M
Description: Based on documentation review and staff interviews, the licensee did not ensure the center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and include all required information.

Evidence: Ten injury reports were reviewed and 4 did not include any future action to prevent recurrence, 5 did not include the date parent was notified of the injury and 2 did not include the time parent was notified of the injury.

Plan of Correction: Effectively immediately the Center Director will ensure the reports are filled completely.
