A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Standard #: 22VAC40-665-520-B-11
Description: Based on documentation review, the vendor did not obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

The record for Child #2 indicated the child had a food allergy plan. The record did not contain a written allergy care plan. A member of management confirmed this information during the inspection.

Plan of Correction: We will have the parent submit an allergic action plan with the required information.

Description: Based on documentation review and inteview, the vendor did not ensure two staff members had been issued a statement that staff were determined to be free of communicable tuberculosis (TB). The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence:
The record for Staff #2 contained a TB screening dated 02/21/2020. The documented hire date for Staff #2 was 02/19/2020. The record for Staff #5 contained a TB screening dated 12/11/2019.
