The inspector conducted a mandated monitoring inspection from 1:15 until 2:30 p.m. There were 16 infants and children in care with staff supervising at all times. Children were observed during free play, hand washing, being read to, and diapering. Five children's records were reviewed and five staff records were reviewed. No medications were reported to have being administered. An acknowledgement form was signed by the director; the inspection documents will be sent. Standard #: 22VAC40-185-270-A Description: Based on observation, the center did not maintain the building inside in a safe condition.
Evidence:
1. The Inspector observed a pool noodle on the indoor slide which is coming apart presenting a choking hazard.Plan of Correction: The noodle will be replaced today.
Standard #: 63.2(17)-1721.1-B-2 Description: Based on record review and interview, the center did not have fingerprint record checks in the file prior to the 1st day of employment.
Evidence:
1. The inspector reviewed five staff records. Two of five staff records (#2, #3) did not have fingerprint checks completed prior to the first day of employment. They were in the file, but not before the first day of employment.Plan of Correction: The director now knows that the fingerprints checks have to be completed prior to the first day of employment.
