Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 10:15 AM and departed at 1:40 PM. Twenty-one children were in care. A sample size of three staff records and five children's records were reviewed. No children's injuries were reported and it was reported that no medication is administered to children in care.

An exit meeting was conducted with the program director prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based upon review of five children's records, the facility has not ensured that each child has a physical examination by or under the direction of a physical prior to attending or within one month after attending.
Evidence:
The record for child 4 indicates that child 4 began attending on 11/18/2019. The record did not include documentation of a physical examination.

Plan of Correction: The facility responded with the following:
All children's records will be checked to ensure that each record includes documentation of a physical examination. Infant parents will be informed that the physical examination is quite often called a well baby exam.

Standard #: 22VAC40-185-280-B
Description: Based upon observation, the facility has not ensured that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. In addition, the facility has not ensured that pesticides are kept in separate storage areas from cleaning supplies.
Evidence:
1. The child proof latch on the tall storage cabinet in the far corner in the back room was not locked. In this cabinet are numerous cleaning, janitorial, and repair products. There are there gallon containers of paint and two containers of spray paint labeled "caution, keep out of reach of children". Also in the cabinet was a gallon container of clorox bleach and two aerosol cans of Hot Shot insecticide labeled "warning, keep out of reach of children". The cans of Hot Shot were on the same shelves as the Cloros, Fbauloso and other cleaning products.
2. The high wall-mounted cabinet in the foyer was unlocked. There was a container of Febreeze labeled "caution, keep out of reach of children" in the cabinet.
3. The door to the director's office does not lock. There was a container of anntibacterial wipes and a container of hand sanitizer, both labeled "caution, keep out of reach of children" on the file cabinet and desk.
4. There was a container of hand soap labeled "caution, keep out of reach of children on the sink in an unlocked bathroom.

Plan of Correction: The facility responded with the following;
We will scour the facility to make certain that all potentially hazardous substances are kept in locked cabinets/closets at all times. All staff will be reminded of this responsibility.
Insecticides/pesticides will not be kept at the facility.
