Comments:
An unannounced subsidy health and safety inspection was conducted on 02/21/2020 from 10:45 am - 2:40 pm. A monitoring inspection was concurrently conducted as this facility is a licensed child day center. At the time of entrance there were 45 children in care with 8 teaching staff and two administrative staff. The children in care ranged in age from infant - five years. Children were observed during morning program time, lunch and nap. Records were reviewed for ten children and six staff.

Violations:
Standard #: 22VAC40-665-500-F
Description: Based on record review, the vendor failed to maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.

Evidence:
1. Child 1 has a diagnosed food allergy to tree nuts and peanuts. This allergy was not included on the food allergy list maintained in the kitchen and child 1's classroom.

Plan of Correction: Food Allergy List has been updated and will be maintained monthly or as new students enroll with allergies. Management and Safety Captain will monitor for compliance.

Standard #: 22VAC40-665-500-E
Description: Based on interview, the vendor failed to inform all staff who work with children of children's allergies, sensitivities, and dietary restrictions.

Evidence:
1. Staff 1, who prepares food for the facility, was not aware that child 1 had a diagnosed food allergy to peanuts and tree nuts.
a. Staff 1 stated she was aware of child 1's allergy to another food however, she did not know that child 1 had an additional food allergy to tree nuts and peanuts.

Plan of Correction: Child one's allergy will be added to the food allergy list and parent will provide proper documentation by 03/06/2020

Standard #: 22VAC40-665-510-B
Description: Based on record review and interview, the vendor failed to maintain a written hard copy record of daily attendance that documents the arrival of each child in care as it occurs.

Evidence:
1. The arrival time for child 2 was not listed on any of the attendance records maintained by the vendor when viewed at 10:55 am.
2. Administrative staff confirmed that child 2 arrived at approximately 9:45 am and the arrival time had not been logged into the electronic attendance system nor had his arrival been noted on the hard copy of the classroom attendance sheet.

Plan of Correction: Face-to-Name sheet procedures will be reviewed staff by 3/6/2020. Management will verify face to name sheets multiple times throughout the day to ensure compliance. DM will inspect on visits.
