Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, February 28, 2020. The on-site inspection was conducted from 8:10 a.m. to 11:20 a.m. On this date, there were 18 children in one classroom and in the care of two staff members. There were a total of 20 children enrolled in the preschool program. Five children's records were reviewed. The staff records were reviewed on September 16, 2019. Two staff members and three children were interviewed. The medication and medication paperwork were reviewed for one child. Breakfast was served by the elementary school and was observed to meet USDA guidelines. The theme for the week was Pets and the children enjoyed making pet habitats. Some of the items observed and reviewed included: current health and fire inspections, required posted information, documentation of emergency evacuation and shelter-in-place drills, first aid kit and emergency supplies, injury reports, opening circle time, breakfast, hand washing and bathroom procedures, transitions, center time, and outdoor play.

Description: Based on observation and confirmation with staff, the center failed to have a working battery-operated radio on-site.

Evidence:
1. The radio to be used in case of emergency was not in working order.
2, Staff A confirmed the radio as not working.
