Comments:
2/27/20 - Licensing Inspector arrived at 7:15 and departed at 8:00 AM to conduct the field visit portion of the renewal. At the conclusion of the inspection, there were 26 school-age children present with three staff. Children were observed participating in an activity designing their own amusement park attraction. Medication was reviewed. The sample size was five children's records.

2/28/20 - Licensing Inspector arrived at the AlphaBEST office at 8:30 and departed at 10:00 AM to review staff records. The sample size was five staff records.

Violations:
Standard #: 22VAC40-185-240-D-5
Description: Based on review of five staff records, it was determined that the facility did not ensure that there was always at least on staff member on duty that had current daily health observation training.

Evidence:
1. AlphaBEST management indicated that staff #1, staff #2 and staff #5 are present at the facility for after school care.
2. None of the identified staff had documentation of obtaining daily health observation training.

Plan of Correction: AlphaBEST management will ensure that staff obtain daily health observation training.
