Comments:
A Subsidy Health and Safety inspection was conducted on 2/24/2020 to review supplemental health and safety requirements. There were eight children in care when the inspector arrived.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-520-B-11
Description: Based on review of five children's records, the vendor failed to ensure that all diagnosed food allergies had a written action plan.

Evidence:
1. Child # 4 has a diagnosed food allergy and did not have written allergy care plan completed by the physician.

Plan of Correction: During the inspection the owner printed a blank form and will have the parent to take to the doctor's office.

Standard #: 22VAC40-665-650-E
Description: Based on interviews and procedures review, the vendor failed to ensure that a they developed and implemented a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members.

Plan of Correction: The owner and director will develop the policies and will ensure that all staff are aware.

Standard #: 22VAC40-665-770-B-5
Description: Based on review of the emergency plan, the vendor failed to ensure that they developed a plan for continuity of operations procedures to ensure that essential functions are maintained during an emergency.

Evidence:
1. The emergency plan did not have the procedure for continuity of operations during an emergency.

Plan of Correction: The director and owner will ensure that the procedure is developed and added to the emergency plan.
