An unannounced monitoring inspection was conducted today 10/18/2019. The inspection began at 8:40 am and concluded at 11:30 a.. There were a total of 30 children present with six staff members. During the inspection five children's, and six staff's records were viewed, two buildings were viewed, emergency supplies were viewed, infant care was observed, diapering was observed, food service was viewed, emergency supplies were viewed and polices and procedures were reviewed. The center does not administer any medication per policy and does not provide any transportation.

Standard #: 22VAC40-185-500-A Description: Based on observation, the center failed to ensure that all hand washing procedures were followed as required by the standard.
Evidence:
1. Observed in the infant room the staff did not wash their hands prior to fixing and feeding a bottle to child # 4. The staff had gloves on when preparing the bottle and feeding the bottle.
2.The child's hands were not washed prior to being fed the bottle.Plan of Correction: The staff will make sure that all hands are washed as required.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of six staff's records, the center failed to ensure that all CPS background checks were completed and returned within the 30 days hire.
Evidence:
1. Staff # 1 was hired on 12/3/2019 and on the day of the inspection the CPS background checks had3a not been returned. There was no documentation of contact with OBI in the staff's record.
2. Staff # 6 was hired on 1/11/2020 and no documentation of the CPS background check in the record on the day of the inspection.Plan of Correction: The director stated that they called OBI today and were told the system was down. In the future all backgrounds will be monitored and four days after the 30th day of hire OBI will be contacted and the results will be documented in the staff's record.
