Technical Assistance:
We discussed the requirements for children's records and emergency drill records. The risk assessments for violations were provided.

We talked about options for ensuring adequate gross motor activities and the playground use schedule.

Comments:
Thank you for your assistance during the unannounced monitoring inspection conducted from 3:03 PM to 6:05 PM. Today, there were thirty-two school age children with five to six staff. I viewed program activities, snack, nutrition, equipment, daily routines, staffing, supervision, interactions with children, injury records, agency reports, emergency supplies and drill records, emergency plan, five records for children, four staff records, staff training, indoor and outdoor areas. The children enjoyed table activities, computer use, indoor games and playground time. Volunteers have been regularly present to assist with program activities. The staff were observed providing frequent interactions with children and close supervision.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of five children's records, and an interview with administrative staff, the program failed to obtain documentation of immunizations prior to initial attendance by each child. Evidence: Child 1, 2 3 and 5 did not have an immunization record in the file.

Plan of Correction: Administrative staff has worked with school staff and with parent permission for the release of copies of immunization records. The immunization records will be obtained for the four records. Administrative staff will work with new parents and school staff to ensure that parents understand the information required before initial attendance.

Standard #: 22VAC40-185-140-A
Description: Based on a review of five records for children, and an interview with administrative staff, the program failed to obtain physical examination records for all children within thirty days of initial attendance. Evidence: Child 1 started on September 20, 2019 and there was not a physical examination report in the file. Child 2 began attending on October 2, 2019 and there was not a physical examination report in the file.

Plan of Correction: The administrative staff will work with the parents and school staff to obtain a release of a copy of medical records so that the required information is obtained. The director will make sure the required physical examination records are obtained within thirty days if they are not provided prior to initial attendance.

Standard #: 22VAC40-185-550-E
Description: Based on a review of emergency practice drill records, and an interview with administrative staff, the program failed to ensure that all drills were maintained in a written record. Evidence: The section for documenting the evacuation drill for January 2020 was blank. There was information for the prior month on December 13, 2019 and the current month's practice on February 3, 2020. Staff confirmed that the drill was practiced and not written in the record. One administrator received phone communication the day of the drill that has been deleted.

Plan of Correction: The site director responsible for conducting emergency drills will make sure all information is written on the drill form at the time of the practice.
