Comments:
An announced modification inspection was conducted on Wednesday, February 26, 2020 from 10:10am until approximately 10:50am. The Department received a written request to modify the facility?s license on December 30, 2019. The licensee has requested a modification to the facility?s location. The licensing inspectors completed an inspection of the classroom, hallways, bathrooms, and office. The inspectors measured the child care spaces. The building?s occupancy, fire inspection, health inspection, asbestos report, and policies and procedures have been reviewed. At this time, the licensing inspector recommends approving the modification request.

Standard #: 22VAC40-185-260-C
Description: Based on observations at the facility on 02/26/2020, the facility did not post a notice regarding the presence and location of asbestos containing materials and advising that the asbestos inspection report and management plan are available for review.

Evidence: 1) A notice regarding the presence and location of asbestos containing materials was not observed at the facility during the inspection.

Plan of Correction: The notice will be posted today.

Description: Based on observations at the facility on 02/26/2020, the facility did not ensure that a daily schedule was posted.

Evidence: 1) There were no daily schedules posted at the facility.

Plan of Correction: A copy of the daily schedule will be posted today.

Standard #: 22VAC40-185-540-A
Description: Based on observations at the facility and interview on 02/26/2020, the facility did not ensure that a first aid kit was available on each floor of each building used by children.

Evidence: 1) The licensing inspector did not observe a first aid kit in the building. 2) During interview, a member of management reported a first aid kit was not available in the building during the inspection.

Plan of Correction: The first aid kit will be purchased and brought to the facility today.

Standard #: 22VAC40-185-540-E
Description: Based on observations at the facility and interview on 02/26/2020, the facility did not ensure to have the required nonmedical emergency supplies.

Evidence: 1) The licensing inspector did not observe a battery-operated flashlight or a battery-operated radio in the building. 2) During interview, a member of management reported the items were not available in the building during the inspection. The following non-medical emergency supplies shall be required: one working, battery-operated flashlight on each floor of each building that is used by children; and one working, battery-operated radio in each building used by children and any camp location without a building.

Plan of Correction: The items will be brought to the facility today.

Description: Based on interview, the facility's emergency preparedness plan did not meet all of the required requirements.

Evidence: 1) During interview, a member of management reported the emergency preparedness plan for the previous location would be used for the new proposed locality. The plan has not been developed in consultation with local or state authorities for the new locality. The center shall have an emergency preparedness plan that addresses staff responsibility and facility readiness with respect to emergency evacuation and shelter-in-place. The plan, which shall be developed in consultation with local or state authorities, addresses the most likely to occur emergency scenario or scenarios, including but not limited to natural disaster, chemical spills, intruder, and terrorism specific to the locality.

Plan of Correction: The emergency preparedness plan will be reviewed with local authorities as soon as possible.

Description: Based on observations at the facility on 02/26/2020, the facility did not ensure emergency evacuation and shelter-in-place procedures/maps were posted in a location conspicuous to staff and children on each floor of each building.

Evidence: 1) The licensing inspector did not observe evacuation and shelter-in-place procedures/maps posted in the building.

Plan of Correction: The evacuation and shelter-in-place procedures/maps will be brought to the facility today.
