Discussed with Head of Lower School: In addition to fire drills and shelter-in-place drills being practiced during the school year, fire drills and shelter-in-place drills need to be practiced during after school hours of operation and when summer camp is in session, each program should keep a record of drills practiced during their hours of operation.

An unannounced monitoring inspection was conducted today from 11:00am-2:20pm. There were 34 children (3yrs -5yrs.) attending the preschool program directly supervised by 5 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, playing matching games with cards, doing crafts, playing with blocks and with play-doh, listening to a story, playing on the playground and napping. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Evidence:
The injury record dated 12/9/2019 is missing information about future actions to prevent recurrence of the injury. The injury record dated 2/3/2020 is missing the time when parents were notified.Plan of Correction: We will review protocol with staff to make sure every field is filled out.
