Comments:
A Subsidy Health and Safety inspection was conducted on 2/27/2020 to review supplemental health and safety requirements. Licensing Inspector arrived at 7:15 and departed at 8:00 AM to conduct the field visit portion of the renewal. At the conclusion of the inspection, there were 26 school-age children present with three staff. Children were observed participating in an activity designing their own amusement park attraction. Medication was reviewed. The sample size was five children's records. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

2/28/20 - Licensing Inspector arrived at the AlphaBEST office at 8:30 and departed at 10:00 AM to review staff records. The sample size was five staff records.

Violations:
Standard #: 22VAC40-665-580-E-1
Description: Based on review of five staff records, it was determined that the vendor did not ensure that within 90 days of employment all staff had on file current certification in cardiopulmonary resuscitation (CPR)

Evidence:
The following staff, all of whom had been employed over 90 days, did not have documentation of current CPR on file:
- Staff #1 (date of hire 9/3/19)
- Staff #4 (date of hire 9/3/19)
- Staff #5 (date of hire 9/3/19)

Plan of Correction: AlphaBEST management will ensure that all staff have current CPR documentation on file.

Standard #: 22VAC40-665-580-E-2
Description: Based on review of five staff records, it was determined that the vendor did not ensure that within 90 days of employment all staff had on file current certification in first aid.

Evidence:
The following staff, all of whom had been employed over 90 days, did not have documentation of current first aid on file:
- Staff #1 (date of hire 9/3/19)
- Staff #4 (date of hire 9/3/19)
- Staff #5 (date of hire 9/3/19)

Plan of Correction: AlphaBEST management will ensure that all staff have current first aid documentation on file.
