Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22VAC40-191 (Background checks); 22VAC40-185-(3)-240 (Training)

An unannounced monitoring inspection was conducted on 2/25/20 from 3:15pm - 4:45pm. During the inspection there were eight children ages 10 months old through eleven years old in care with two staff. Children were observed participating in various activities in the classrooms, eating snack and during rest period. Records were reviewed for five children and three staff during the inspection. There was no medication at the facility. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Standard #: 22VAC40-185-160-C
Description: Based on a review of three staff records, it was determined that the facility did not ensure that at least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.

1. The record for staff #1, contained documentation of TB screening that was dated 1/15/18.
2. Staff #1 confirmed that she had not completed an updated TB screening.

Plan of Correction: The facility responded: Staff #1 will complete an updated TB screening.

Standard #: 22VAC40-185-240-C
Description: Based on a review of documentation and interview, it was determined that the facility did not insure that all staff who work directly with children shall annually attend 16 hours of staff development activities that shall be related to child safety and development and the function of the center.

1. The records staff #1 and staff #2 did not include documentation to demonstrate either staff had completed the required 16 hours of annual training.
2. Staff #1 confirmed that neither staff had completed all of the 16 hours of training required annually.

Plan of Correction: The facility responded: We will ensure that all staff complete the required 16 hours of training annually.

Standard #: 22VAC40-185-560-F
Description: Based on observation and interviews, it was determined that the licensee did not ensure that when the center chooses to provide meals or snacks, a menu listing foods to be served for meals and snacks during the current one week period should be dated.

1. The menu posted for the current week did not list an afternoon snack for today, 2/25/20
2. Staff #1 confirmed that there was no snack listed for the afternoon of 2/25/20.

Plan of Correction: The facility responded: We will ensure that a menu which includes all of the meals and snack provided by the center for the children in care.
