Comments:
An unannounced monitoring inspection was conducted on 01/15/2020 in response to a self-report incident that happened on 01/08/2020. Interviews were conducted with staff, documents were reviewed and staff/children records were reviewed. Areas of non-compliance are listed on the Violation Notice. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review, it was determined that staff members did not obtain and submit the results of a follow-up tuberculosis (Tb) screening at least every two years as required.
Evidence:
1) The record for Staff 1 did not contain documentation of a current Tb screening result. The most recent Tb screening was dated 09/14/2017.

Plan of Correction: A new Tb screening was completed on 01/20/2020.

Standard #: 22VAC40-185-270-A
Description: Based on documentation review and staff interviews, it was determined that areas and equipment of the center were not maintained in a clean, safe and operable condition.
Evidence:
1) On 01/08/2020, Child 1 (2 years old) pushed the glass of the front door of the school and was able to open the door and exit the school.
2) Staff 4 stated that "about a year ago, a new door was installed and since then, the security mechanism (exit button) that prevents the door from being opened if pushed by children had been deactivated".

Plan of Correction: A work order to fix the security mechanism on the door was placed on 01/08/2020. Door was fixed on 01/10/2020.

Standard #: 22VAC40-185-570-A
Description: Based on observation and staff interview, it was determined that infants placed in high chairs did not have protective safety belts securely fastened.
Evidence:
1) On 01/15/2020, in Infant Room 3, Staff 3 placed Child 2 and Child 3 in high chairs to eat but did not buckle the security belts of the high chairs.

Plan of Correction: Corrected during the inspection.
