2/24/20 - Licensing Inspector arrived at 6:45 and departed at 8:00 AM to conduct the site inspection part of the renewal. At the conclusion of the inspection, there were 28 school age children present with two staff. Children were observed participating in free play, working with clay and doing large motor activities in the gym. There is no medication being stored or administered at the facility. The sample size was five children's records.
2/18/20 - Record review for renewal conducted at AlphaBest office. The sample size was two records.

Standard #: 22VAC40-185-60-A Description: Based on review of five children's records, it was determined that the facility did not ensure that all required elements were present in each child's record.
Evidence:
The record for child #5 (date of enrollment 10/23/19) did not include an address for the two emergency contact persons.Plan of Correction: Site director will ensure that addresses are obtained while continuing to audit childen's records for completeness.
Standard #: 22VAC40-185-260-A Description: Based in review of documents at the center, it was determined that the center did not have an annual fire inspection available for review.
Evidence:
The fire inspection posted at the facility was dated 10/27/18 and was, therefore over a year old.Plan of Correction: Site director and AlphaBEST management will work with the school to obtain an updated fire inspection.
Standard #: 22VAC40-185-550-D Description: Based on review of documents posted at the facility, it was determined that the facility did not ensure that it implemented a monthly practice evacuation drill.
Evidence:
There was not an evacuation drill documented for December of 2019.Plan of Correction: Site director will ensure that all required emergency drills are conducted and documented on a monthly basis.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of two staff records, it was determined that the facility did not ensure that each staff member had a completed search of the central registry (CPS check) on file within 30 days of employment.
Evidence:
The record for staff #1, date of hire 10/1/19, did not include a completed CPS check.Plan of Correction: AlphaBEST management will ensure that all staff have completed CPS checks on file within 30 days of employment.
