Technical Assistance:
The fire department inspection is now due. Please contact the school administration or the fire department itself to obtain the inspections.

Comments:
An unannounced, mandated renewal inspection was conducted at the site on 2/20/2020. The inspector arrived at 7:15 AM and departed at 8:15 AM. Twenty-one children were in care with two staff. Morning arrivals, activities and preparation to go to public school were observed. A sample size of five children's records were reviewed. Medication administration was reviewed. No children's injuries were reported.
Three staff records were reviewed on 2/25/2020.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based upon review of three staff records, the facility has not ensured that each staff submits results of a negative tuberculosis screening at least every two years.
Evidence:
The facility was unable to provide documentation of a tuberculosis screening within the past two years for staff 2.

Plan of Correction: The facility responded with the following:
If staff 2 is unable to produce documentation of a tuberculosis screening, she will be required to obtain a new one. In the future, tuberculosis screenings will be tracked and staff sent as required.

Standard #: 22VAC40-185-50-A
Description: Based upon observation, the facility has not ensured that staff and children's records are treated confidentially.
Evidence:
The Supplemental Information Form containing names of the children was posted on the parent information board. This form contains confidential information and includes a written directive on the form indicating that the form is confidential and is not to be posted.

Plan of Correction: The facility responded with the following:
The Supplemental Information form was removed from the parent board during the inspection. Supplemental Information sheets will not be posted in the future.

Standard #: 22VAC40-185-550-D
Description: Based upon review of emergency drill documentation and staff interview, the facility has not ensured that two shelter-in-place drills for the most likely scenarios are practiced a minimum of two times per year.
Evidence:
1. The emergency drill record provided did not include documentation of any shelter-in-place drills in the last year.
2. Staff 1 stated that she thought the lock down drills were what were required.

Plan of Correction: The facility responded with the following:
Evacuation drills will be practiced and documented monthly. At least two shelter-in-place drills will be practiced and documented per year. At least one lock-down drill will be practiced and documented annually.

Standard #: 22VAC40-185-560-F
Description: Based upon observation and staff interview, the facility has not ensured that a menu listing foods to be served for meals and snacks during the current one-week period is posted.
Evidence:
1. There was no menu posted for the current week - 2/17/2020 - 2/21/2020.
2. Staff 1 stated that the menu had not been given to them for the week.

Plan of Correction: The facility responded with the following:
In the future the menu for each week will be posted by Monday morning.
