Comments:
A Subsidy Health and Safety inspection was conducted on 02/19/2020 from 8:50 A.M.-11:30 A.M., and on 02/26/2020 from 8:55 A.M.-10:45 A.M. to review supplemental health and safety requirements. At the time of inspection 23 children were in the care of four staff. six children records, five staff records, six agent background checks, one medication and required documentation, fire inspection, first aid kits, emergency supplies, evacuation drills, health inspection, playground, and posted required information were reviewed. Children were observed during free choice activities, handwashing, story time, center time, special guest speakers in the Pre-K classroom, an art activity, behavioral guidance, and transition to lunchtime. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. The exceptions to compliance are noted on the violation notice.

Standard #: 22VAC40-665-520-B-11
Description: Based on review of medication, the center failed to have a written allergy plan for each child diagnosed with a food allergy.

Evidence:
1. Child 1M had a a emergency medication at the center. There was not a written allergy plan for the child at the center.
2. Staff verified that there was not an allergy plan on site.

Plan of Correction: Administration contacted the parents and requested a written allergy plan be obtained from the doctor when they bring the medication back to the center.

Standard #: 22VAC40-665-780-A-3
Description: Based on a review of evacuation drills, the center failed to ensure that one lock down shall be practiced at least annually.

Evidence:
1. There was no documentation of a Lock Down drill be completed for the calendar year 2019.
2. Staff verified that a lock down drill had not be completed for the calendar year 2019.

Plan of Correction: Administration will ensure that a lock down drill is completed for the calendar year 2020.
