Consultation was given on completing the injury form with a "real time" rather than pick-up for accuracy.
An unannounced renewal study was conducted today. Fifteen children were present with two staff and within ratios for children ages 3-5. Observed transition from lunch (bought from home) to hand washing and using the bathroom, reading and puzzles and preparation to go outside to the large center playground. Children wear yellow shirts to help identify them while outside as a large number of community kids also use the playground. Reviewed training and background checks for staff on 2/7/2020 off site at a central location. Areas of non-compliance are identified in the violation notice. Child #1 was missing an annual update (signed 8/24/2018) and documentation that proof of identity was viewed.

One form was missing a second signature and one form was missing the time the parent was notified.
Plan of Correction: Moving forward, forms will be completed and ensure that the time is documented.
