An unannounced monitoring inspection was conducted from 11:50 am-4:00 pm. At the time of entrance, 28 children were in care with 5 staff members present. Children were observed during nap time, free play and celebrating a classmate's birthday. Handwashing was also observed. A selection of staff and children records, the physical space, evacuation drills, attendance records, injury reports, medications and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. A Subsidy Health and Safety Inspection (SHSI) was also completed on this date. Standard #: 22VAC40-185-140-A Description: Based on record review, the center did not obtain documentation of a physical examination by or under the direction of a physician within one month after attendance for one child.
Evidence:
Child #1 began attending the child day center on 8/12/2019. There was no documentation of a physical exam in the record for Child #1.Plan of Correction: I will ask the parents for a physical today.
Standard #: 22VAC40-185-550-M Description: Based on record review, 3 of 4 injury reports did not contain all required information.
Evidence:
1. An injury report for Child #2 dated 1/2/20 did not contain the time of injury, date and time parents were notified, and documentation on how parent was notified.
2. An injury report for Child #3 dated 12/12/19 did not contain the date and time parents were notified, documentation on how parent was notified, any future action to prevent recurrence of the injury, and staff and parent signatures or two staff signatures. The injury report contained only one staff signature.
3. An injury report for Child #4 dated 11/19/19 did not contain any future action to prevent recurrence of the injuryPlan of Correction: I spoke with staff about the information missing. Copies of the most up to date injury report form were made and placed in the folder for staff.
