Comments:
A Subsidy Health and Safety inspection was conducted on 2/27/20 to review supplemental health and safety requirements. 2/27/20 - Licensing Inspector arrived at 6:30 and departed at 7:15 AM to conduct the site visit portion of the renewal. At the conclusion of the inspection, there were 21 preschool and school-age children (in two groups) with three staff. Children were observed playing with legos. There was no medication on site. All required paperwork was reviewed. The sample size was five children's records. On 2/28/20, the licensing inspector conducted a staff record review at the AlphaBEST office. The sample size was three staff records. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. No violations of the supplemental subsidy health and safety requirements were found.

Description: Based on review of three staff records, it was determined that the vendor did not ensure that all staff documentation of current cardiopulmonary resuscitation (CPR) on file within 90 days of employment.

Evidence:
Staff #1 (date of hire 9/4/19) and staff #4 (date of hire 9/4/19) did not have documentation of current CPR on file.

Plan of Correction: AlphaBEST management will ensure that all staff have documentation of current CPR on file.

Standard #: 22VAC40-665-580-E-2
Description: Based on review of three staff records, it was determined that the vendor did not ensure that all staff documentation of current first aid on file within 90 days of employment.
