Comments:
An unannounced renewal inspection was conducted Friday, February 7, 2020 to determine the center's compliance with licensing standards. The inspector arrived at 9:45 AM and concluded the inspection at 2:06 PM. The census for today's inspection consisted of 28 children. During the inspection, children were having story time, singing songs, taking bathroom breaks, washing hands, eating lunch and napping. Children's equipment, learning materials, indoor and outdoor space, restrooms, first aid kit and the nonmedical emergency supplies were reviewed. Medication is administered and medication, authorization forms and allergy plans were reviewed. The provider stated that transportation is provided and the vehicle and transportation documents were reviewed. Five children's records and five staff records, annual fire and health inspections, fire drill, lockdown drill and shelter in place drill logs, daily attendance and all required postings were reviewed.

The director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that all hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In classroom #1, one container of Pine-Sol multi-surface cleaner labeled "Hazard to humans and domestic animals," "Keep out of reach of children," and "Caution" and one container of Bleach labeled "Keep out of reach of children" and "Danger" were in an unlocked cabinet accessible to children.
2. In classroom #3, one container of Lysol wipes labeled "Danger" and "Keep out of reach of children" were unlocked on a shelf accessible to children.
3. In classroom #4, one container of disinfecting wipes labeled "Caution" and "Keep out of reach of children"; seven cans of paint labeled "Keep out of reach of children," "Hazard to humans and domestic animals" and "Caution"; one container of Patching Plaster labeled "Warning" and "Keep out of reach of children" and one container of Goo-Gone labeled "Flammable" and "Keep out of reach of children" were in an unlocked closet accessible to children.

Description: Based on observation, the center did not ensure that staff washed their hands with soap and running water after coming in contact with body fluids.

Evidence:
In classroom #1, a staff member was observed changing a child's diaper. After the staff member changed the child's soiled diaper, the staff member did not wash their hands with soap and running water.

Description: Based on observation, the center did not ensure that a nonabsorbent surface for diapering and changing shall be used.

Evidence:
In classroom #2, the diaper changing pad was torn in three places and was no longer nonabsorbent.

Description: Based on observation and an interview, the center did not ensure that foods that are considered a potential choking hazard were not served to children three years of age or younger.

Evidence:
1. In classroom #2, children were served whole hot dogs.
2. In classroom #2, children are under three years of age.
3. The director stated that she usually cuts the hot dogs up but forgot to cut them today.

Description: Based on observation, the center did not ensure all prepared infant formula was dated and labeled with child's name.
