Technical Assistance:
Inspector and director discussed staff files, children's records, background checks, emergency procedures, fire inspection, and developmentally and age appropriate materials.

Comments:
An unannounced monitoring inspection was conducted today from 10:10am-12:45pm. There were 47 children directly supervised by 8 staff. The physical plant, 4 staff records, 3 children?s records, evacuation drills, and policies were inspected. Children were observed participating in group play, reading books, playing outside, singing and dancing, participating in Spanish class and walking to and from the classroom. There was a supply of materials for the children. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not maintain all required information in children's records.

Evidence:
1) Child 1 is missing two street addresses for both emergency contacts.

Plan of Correction: We will follow up with the parents and obtain the information.

Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment in the center was not maintained in a safe condition.

Evidence: In a crib, a stuffed bear was found to be close to the head of a sleeping infant. The American Academy of Pediatricians recommend no stuffed toys in cribs with infants.

Plan of Correction: The stuffed toy was removed from the crib at the time of inspection .

Standard #: 22VAC40-185-520-C
Description: Based on observation, the center did nor ensure all diaper ointments met all requirements.

Evidence:
1) In the infant room, topical creams were not labeled with the child's name and did not have topical cream authorizations.

Plan of Correction: We will follow up with the parents and get all the forms required. We will label the creams.

Standard #: 22VAC40-185-550-D
Description: Based on record review, the center did not implement a monthly practice evacuation drill.

Evidence: Drills were not conducted in January and February 2020.

Plan of Correction: We will ensure drills are conducted monthly.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the center did not ensure all central registry findings were completed within 30 days of employment.

Evidence: Staff 1 does not have a completed satisfactory central registry finding within 30 days of employment. (Staff 1, DOH 10/2019)

Plan of Correction: We will follow up with the status. If needed, we will complete a new background check.
