Comments:
A Subsidy Health and Safety inspection was conducted from 11:40am-1:00pm to review supplemental health and safety requirements. There were 18 children in the direct care and supervision of 4 staff. A sample size of 4 staff records, 3 children's records, the first aid kit, emergency materials, and the physical plant were reviewed. The children were observed eating lunch from home, watching a movie, cleaning up from lunch, hand washing, drawing, playing games, doing an art project, and playing games on electronic devices.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. If you have any questions, please contact me at shahana.green@dss.virginia.gov .

Plan of Correction: I will schedule a CPR re-certification as soon as possible.
