An unannounced complaint investigation was initiated on March 3, 2020 in response to allegations received by the Division on 1/31/2020, regarding ratio, background checks, and evacuation drills. Based on staff interviews, documentation review, and observation, the preponderance of evidence gathered during the investigation does not support the allegations regarding ratio or background checks. Standard #: 22VAC40-185-550-E Complaint related: Yes Description: Based on staff interview and review of the evacuation drill log, the facility failed to conduct and document evacuation drills monthly as required. Evidence: there were no evacuation drills conducted or documented for 11/2019, 12/2019, or 1/2020. Plan of Correction: An evacuation drill was practiced 2/24/2020 and will be conducted monthly.
