Technical Assistance:
Licensing inspector discussed, toileting procedures, Proof of identification and required display of the state license with the owner.

Comments:
An unannounced monitoring inspection was conducted today from approximately 9:15am-11:40am. There were 70 children in the direct care and supervision of 17 staff. A sample of 7 children?s records and 5 staff were reviewed. The children were observed reading, eating breakfast, learning the map and engaged with staff.Two new staff members have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the owner. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not maintain complete records for each child in care.

Evidence:
1. Child A was missing the father's work phone number.
2. Child B was missing 1st day of attendance.
3. Child C was missing the father's work phone number.
4. Child D was missing previous school attended.

Plan of Correction: Files will be updated today and we will put the first day of enrollment of the first page of the application.

Standard #: 22VAC40-185-70-A
Description: Based on record review, the center did not maintain complete records for each staff member.

Evidence:
Staff C did not have proof of experience on file.

Plan of Correction: I will ask her for the information.

Standard #: 22VAC40-185-80-A
Description: Based on interview and observation, the center did not keep a written record of children in attendance each day.

Evidence:
Staff B stated all of the children were in attendance on 03/02/2020 in the "Montessori 2" room. The "Montessori 2" room attendance for 03/02/2020 recorded 20 of the 24 children in attendance.

Plan of Correction: Completed during inspection. We will complete the attendance at the end of each day.

Standard #: 22VAC40-185-280-B
Description: Based on observation and interview, hazardous substances such as cleaning materials were not kept in a locked place.

Evidence:
Disinfectant wipes, disinfectant spray and odor reducing spray were found in an open cubby located in the "Toddler's" room.
Staff A stated they disinfectant is normally kept in the open cubby.

Plan of Correction: We will put them in a locked cabinet.

Standard #: 22VAC40-185-500-A
Description: Based on observation, children's hands were not washed after using the bathroom.

Evidence:
Two children were witnessed using the bathroom and their hands were not washed afterward.

Plan of Correction: Licensing inspector mentioned it to the staff and the Children's hands were washed.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the findings of the most recent inspection of the facility were not posted.

Evidence:
The violation notice for 02/2019 was posted. The most recent inspection occurred 09/2019.

Plan of Correction: Corrected during inspection.
