Comments:
An unannounced monitoring inspection took place on March 3, 2020. There were 92 children in care from ages 3 months old to 11 years years old with 24 staff providing care and supervision. The inspector reviewed nine children's records and nine staff records during the inspection. The LI also reviewed a sample of four medications. Children were observed during the following activities: breakfast, care for infants, lessons on the days of the week, nap, and story time. The inspector also reviewed emergency drill documentation, first aid kits, the physical plant, and emergency maps. There were discussions about the use of cots as well as emergency maps. The inspection took place from approximately 8:00 AM to 12:45 PM. Violations were discussed at the close of the inspection. Standard #: 22VAC40-185-330-B
Description: Based on observations and discussion with staff, the center failed to maintain the required depth of resilient surfacing as required.

Evidence:

1. Under climbable equipment and at the base of the slide, mulch measured at approximately two inches in depth and in some places, dirt was visible.
2. Staff 8 confirmed that additional mulch was needed.

Plan of Correction: Staff plans to add more mulch to the playground.
