Comments:
An unannounced monitoring inspection was conducted between the hours of 8:00 am and 11:00 am. Upon arrival there were about 70 children in care with 15 staff present. A sample of 2 staff and 5 children's records, in addition to 2 medications. Children were observed in learning centers, being read a story and during circle time. Some preschool children were observed finishing breakfast. Infants were observed sleeping in cribs, during tummy time and being fed.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on observation the center did not ensure all required information was in each staff record.
Evidence:
1. Staff #1 and #2 were listed as program leaders in the file. The director confirmed both staff are not program leaders, but are teacher assistants.
Staff #1 did not have an emergency contact address in the record.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
Both staff records were corrected during the inspection.

Standard #: 22VAC40-185-260-B
Description: Based on a record review and interview, the center did not ensure there was an annual approval from the health department.
Evidence:
The last health inspection was dated 02/07/19. The director confirmed a health inspector has not been out to the facility this year.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
During the inspection the director called the health department. The representative said the health inspector has the center on the list to inspect.

Standard #: 22VAC40-185-280-B
Description: Based on observation the center did not ensure hazardous substances were kept in a locked place, using a safe locking method that prevents access by children.
Evidence:
In the preschool classroom there was a liquid air freshener sitting on top of the children's cubby shelves.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
The air freshener was removed from the classroom during the inspection.
