Comments:
An unannounced renewal inspection was conducted today from 8:45am-11:30pm. There were 80 children (infants-5yrs.) supervised by 14 direct care staff. The physical plant, programming, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in circle time, reading and playing. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Description: Based on observation, all areas of the center are not being maintained in a clean, safe condition.
Evidence:
In the intermediates classroom, peeling and chipped paint is accessible to children.

Plan of Correction: We have a work order in, we should be painted by 4/1/20

Standard #: 22VAC40-185-370-5
Description: Based on observations, 30 minutes of tummy time is not being recorded daily for parents to review.
Evidence:
In the infant room, the 30 minutes of tummy time is not being recorded for parental review.

Plan of Correction: Staff will begin recording the amount of tummy time daily.

Standard #: 22VAC40-185-550-M
Description: Based on review of ten injury reports, four were incomplete.
Evidence:
Required information is missing from injury reports: time and how parents were notified.

Plan of Correction: Review procedures to completing injury reports.
