An unannounced monitoring inspection was initiated on January 22, 2020 from 4:00 p.m. to 6:30 p.m. in response to a self reported incident regarding inappropriate discipline. Additional interviews were conducted by phone and on March 3, 2020 from 3:30 p.m. to 6:00 p.m. Based on observation, staff interviews, and documentation reviewed, there are no violations regarding the self reported incident. Other violations were found during the inspection regarding staff files, ratio, and qualifications. Standard #: 22VAC40-185-160-A Description: Based on review of five staff records, the facility failed to have documentation of a tuberculosis test or screening for each staff within 21 days of employment. Evidence: Staff A (date of hire 11/25/2019), did not get a TB test until 12/22/2019; Staff C (date of hire 12/9/2019) did not have a TB test or screening in the file; and Staff D (date of hire 12/3/2019) did not have a TB test until 1/12/2020.
