Evacuation routes during transportation to/from public school.

Comments:
An unannounced monitoring inspection was conducted on 03/03/2020 from 2:45 pm - 5:05 pm. At the time of entrance there were 55 children, toddler - school age, in care with eight teaching staff and the program director. Children were observed during outdoor play and afternoon snack. Free play was observed in one preschool class. Records were reviewed for eight children and four staff in addition to medication and injury documentation.
Areas of noncompliance are listed on the violation notice and were reviewed with the program director at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review and interview, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.

Evidence:
1. Child 1 lacked documentation of a physical exam.
a. Child 1 has an enrollment date of 09/13/2019 and was in care on the date of the inspection.
2. The program director confirmed that a documented physical was not on file for child 1.

Plan of Correction: 1. The child's father was called and informed. The father said that his son had a doctor's appointment and would get an updated physical form and give us a copy for the child's file.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that equipment of the center shall be maintained in a safe condition.

Evidence:
1. The sharp point of two nails was observed protruding from the wall directly above the toilet in the preschool bathroom.
a. This area of the wall is within reach of children using the bathroom.
2. An area of plastic was broken on the plastic dome window attached to the outdoor play structure on the preschool playground.
a. This area of broken/cracked plastic is a pinch point for children's fingers.

Plan of Correction: 1. Nails have been covered up with sticky tac & tape until they can be removed permanently which is planned for this weekend. 3/4/20

2. Area has been covered up with duck tape until a new equipment structure is discussed and ordered. 3/6/20
