An unannounced renewal study was conducted today. All rooms are in ratio during today's inspection.
4 staff and 5 children's records were reviewed. See Violation notice for any non-compliance. Children were observe resting in all areas. Attendance sheets, tummy time, label and dated bottles, first aid supplies, emergency supplies, medications and emergency escape drills are all in compliance.

Plan of Correction: I am going to put in a 'ticket" to be able to access the references on line. They will be printed, signed and dated.
I will have the staff member fill out a new form.
Standard #: 22VAC40-191-60-B Description: Based on a review of records, one staff record did not include a completed Sworn Disclosure Statement (SDS) prior to the first day of employment.
Staff A's record does not contain documentation of a completed SDS. (Date of Hire: 2/25/2020)
Plan of Correction: A new one will be competed. A form was competed; however, it cannot be located today.
