Comments:
A Subsidy Health and Safety Inspection was conducteed on March 4, 2020 from approximately 12:05 pm-3:30 pm to review supplemental health and safety requirements. Upon arrival to the facility, there were eight children in care with one staff person and one volunteer. The children ranged from 15 months old to three years old. The children were observed watching children's programming, preparing for nap and resting. Exit interview was conducted with the Owner/Program Director.

A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-520-B-3
Description: Based on record review, in two of five children's records reviewed, the vendor did not ensure that all required parent contact information was documented in each child's record.

Evidence: The Director confirmed that the records for Child #2, Child #3, and Child #4 did not include the employment address of each custodial parent.

Plan of Correction: Employment addresses will be corrected.

Standard #: 22VAC40-665-650-E
Description: Based on staff interview, the vendor did not develop and implement a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members.

Evidence: The Director confirmed that there was not a consistent care policy developed for the center.

Plan of Correction: A consistent care policy will be updated and corrected.
