Technical Assistance:
The license cannot be processed for renewal until the complete renewal application and required fee are received.

Comments:
An unannounced full renewal inspection was conducted today. There were 58 children in care, with a total of 6 staff members assigned to their direct supervision. Children's records, field trip administrative forms, the physical plant, emergency preparedness items, staff:child interactions, classroom programming, on site staff records, staff training, and children's medications were inspected today.

The children were observed playing board and card games, coloring pictures, creating artwork, reading books, and interacting with staff members.
The children were expected to depart for off site field trips to local community centers later this afternoon.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of children's health records, it was determined that center staff did not obtain documentation showing that one child has received the immunizations required by the State Board of Health before the child began attending the center.
Evidence:
Child 6 entered care August, 2019. This child's health record does not include documentation of a current immunization record.

Plan of Correction: I will ask the school nurse for a copy of the child's shot record.

Standard #: 22VAC40-185-140-A
Description: Based on a review of children's health records, it was determined that the center did not obtain documentation showing that one child had a physical examination by or under the direction of a physician within one month of the child's attendance.
Evidence:
Child 6 entered care August, 2019. This child's health file does not include documentation of a physical exam.

Plan of Correction: I will ask the school nurse for a copy of the child's physical exam report.

Standard #: 22VAC40-185-560-G
Description: Based on observations, it was determined that two of the children's lunch bags filled with food from home were not labeled as required.
Evidence:
One lunch bag was not dated to indicate the current date. One lunch bag was not labeled to indicate the child's name.

Plan of Correction: Both bags were immediately labeled during the inspection.
