Comments:
The inspector conducted an unannounced mandated renewal inspection from 10:00 a.m. until 12:30 p.m. There were 7 children in care with two staff supervising at all times. The director was also present. Children were observed having snack, playing outside and inside, napping, and being read to. Violations had been corrected from the last inspection. Five children's records and five staff records were reviewed. Please contact your inspector at 276-698-9981 with any questions.

Description: Based on record review, all staff did not have a repeat T.B. test.
Evidence:
1. The inspector reviewed five staff records. One of five records (# 2) did not have a repeat T.B. screening every two years.

Plan of Correction: Staff will get the T.B. test and all staff records will be reviewed.
